Full Time Union City, NJ Posted 16 hours ago $40,643.00 USD / Year NHCAC Project SHAPE is seeking a compassionate, organized, and client-focused Case Manager to provide assessment, care coordination, and supportive services to older adults and individuals in need of community-based assistance. The Case Manager will work directly with clients to evaluate needs, coordinate services, assist with benefit applications, and provide ongoing support to help individuals remain safely and independently in their homes.
Qualifications:
Associate Degree in Psychology or Sociology preferred; will accept High School Diploma Experience in heavy customer service interaction including phone contact relative to the field of Social Services. Ability to work in a multi task environment. Computer literate and quick, accurate entry of information. Bilingual English/Spanish Must be fully vaccinated against COVID-19
