out of 5 stars 1215 3rd Street NW, Albuquerque, NM 87102 From $21 an hour - Full-time HopeWorks 31 reviews From $21 an hour - Full-time The purpose of this position is to provide onsite case management and supportive housing services to tenants of Hope Village. The Supportive Housing Case Manager will be part of a team that will be providing onsite wrap-around services to assist tenants in being successful in housing. These services include building rapport and trusted relationships, conducting home visits, coordinating and ensuring that tenants are engaged in and receiving all necessary support services, and attend and participate in all team meetings. This position will have non-traditional/weekend hours.
ESSENTIAL FUNCTIONS
1. Conducting home visits. 2. Complete all necessary client documentation/billing within the expected timeframe. 3. Assisting clients in understanding their lease and community rules and expectations. 4. Working with property manager and other support staff to address challenges and develop creative and appropriate solutions. 5. Assist in coordination with additional support services and ensure tenant is receiving all necessary services. 6. Create Treatment, Crisis, Discharge and Housing Success Plans. Track progress towards goals. 7. Attend all Hope Village staff meetings. 8. Attend all relevant training courses as directed. 9. Participate as part of a team that will develop and implement relevant on-site activities that will focus on life skills, fostering independence, and enhancing quality of life for tenants of Hope Village.
OTHER RESPONSIBILITIES 1.
Carry out all duties in a manner that is consistent with HopeWorks mission, vision, values and policies. 2. Carry out all duties in a manner consistent with policies and procedures regarding safety protocols as instructed and directed by trained law enforcement officers. 3. Other duties as assigned by relevant agency supervisor.
EDUCATION, EXPERIENCE AND LICENSURE REQUIREMENTS 1.
Bachelor's Degree in Psychology, Sociology, Social Work or related fields preferred. 2. At least one years' experience working with adult SMI populations or within a human services field. Relevant education may be substituted for a portion of experience requirements at the discretion of Hope Works. 3. Experience with HUD/Tenant/Property Management a plus. 4. Current CPR/First Aide certification or willingness to obtain. 5. Must have or be willing to obtain CCSS Certification.
OTHER REQUIREMENTS 1.
Demonstrated ability to work effectively with target population. 2. Excellent interpersonal skills including the ability to work well with a wide variety of people, including clients, staff, colleagues, vendors and individuals throughout the community. 3. Willingness and demonstrated ability to adhere to mission, vision and values in carrying out all facets of job responsibilities. 4. Strong organizational skills 5. Effective time management skills and the ability to effectively manage multiple priorities and to meet deadlines. 6. Ability to work independently display good judgment and problem-solving skills. 7. Good computer skills including knowledge of: Microsoft Office, Word, Excel and the agency's electronic medical record. 8. Must have a valid driver's license, reliable transportation, a good driving record and be insurable by agency's insurance carrier. 9. Bilingual helpful. 10. Must have great attitude.
