Case Managers are to provide short-term financial assistance and case management that primarily focuses on housing stability. Case management consists of costs associated with assessing, arranging, coordinating, and monitoring the delivery of individualized services. Duties and Responsibilities Provide information, referrals, financial assistance, and professional case management for families with dependent children. Case managers will work side-by-side with the client in securing affordable housing. Maintain superior case files with current information, including residency, income verification, and identification documents for all clients in the family unit. Conduct monthly home visits with families and see children at least once every thirty days. Develop housing stability plans with clients, identify their housing barriers, and educate on eviction prevention. Case managers work between the client and landlord and will advocate around leasing-related issues. Respect clients' right to privacy and avoid sharing confidential information; redirect landlords to areas that only relate to the client's lease agreement. Read and be well-informed of the Housing First model and act on its principles in case management. Read and understand HUD's policies and procedures found in 24 CFR Part 578. Case notes and services are to be entered in HMIS in real time. Case notes should be clear, concise, coherent, complete, correct, and conversational. Maintain a caseload of a minimum of 15 clients, no more than 20. Knowledgeable of all available services within the agency and have a full understanding of all current programs and educational classes offered at the agency.
