Are you highly organized, empathetic, and looking to make a meaningful impact in healthcare administration? We are seeking a dedicated Case Management Assistant to join our team. In this vital, unlicensed role, you will serve as the premier frontline point of contact for our residents and their families. You will bridge scheduling gaps, manage department logistics, and act as a supportive liaison between our facility, our residents, and insurance providers. Wage Commensuration:
The wage range for this role takes into account the wide range of factors that are considered in making compensation decisions including, but not limited to, skill sets, experience, education and training, licensure and certifications, and other business and organizational needs.
Duties & Responsibilities:
As a Case Management Assistant, your day-to-day will focus on administrative excellence, compliance tracking, and exceptional customer service:
Frontline Support:
Act as the first point of contact for the Case Management department, warmly fielding and routing questions or concerns from residents, families, and staff.
Calendar & Meeting Coordination:
Manage the department calendar, schedule care conferences, and organize calls with insurance companies.
Documentation & Minutes:
Attend insurance and facility meetings to take detailed notes, record minutes, and track follow-up action items.
Compliance Tracking:
Prepare, deliver, and track essential insurance compliance documentation, ensuring timely delivery and signature collection for Notice of Medicare Non-Coverage (NOMNC) forms.
Discharge Logistics:
Partner with Case Managers and Social Work staff to handle discharge logistics, including compiling discharge packets, sending faxes, and arranging patient transportation.
Referral Intake:
Receive and document preliminary data for new insurance referrals to assist the clinical team with the admission process.
Note:
This is an administrative, unlicensed role. This position does not perform clinical assessments, perform screening evaluations, negotiate contracted insurance rates, or authorize medical orders.
Qualifications & Requirements:
We are looking for someone who is organized, detail-oriented, and passionate about helping others.
Required:
High School Diploma or equivalent (GED). Strong communication skills and a professional, empathetic demeanor. Excellent organizational and multitasking abilities. Preferred (But Not Required): Associate's or Bachelor's degree in Healthcare Administration, Social Work, Business, or a related field. Previous administrative, clerical, or customer service experience in a healthcare or skilled nursing facility setting.
