Job Description

Position Overview The Case Coordinator will work in REMSA's Mobile Integrated Health (MIH) high utilizer team. The MIH team is an interdisciplinary team that meets patients where they are to identify barriers in care and address unmet medical and social needs. The MIH Case Coordinator will be responsible for conducting both mobile visits and performing office-based administrative tasks. During mobile visits, the MIH Case Coordinator will participate in assessments and facilitate service connections/referrals for MIH patients. Office-based administrative tasks will involve participating in performance measures, reviewing policies and educational content to advance the performance and function of the MIH team. Within this general framework, this position is accountable for the following functions: Coordinate and develop individualized care plans. Coordinate referrals to address an individual's social determinants of health, medical, and mental health needs. Engage patients through in-person, telephonic, and virtual outreach. Support continuity of care following EMS or emergency department utilization. Provide a catalyst for absolute professionalism, integrity, confidentiality, and competency with patients, their families, the public, other agencies, coworkers, and REMSA management. Work collaboratively with a Community EMT and other staff to conduct joint visits and case management. Participate in case conferences, multidisciplinary team meetings, and care planning discussions with community partners. Build and maintain partnerships with hospitals, clinics, behavioral health providers, and community-based organizations. Drives and operates a Company vehicle safely, in accordance with established driving guidelines. Assist in the development of monthly and quarterly reports. Provide education to EMS personnel on behavioral health, social determinants of health, and available community resources. Participate in program development and workflow improvement. Responsible for establishing and maintaining good relationships with both internal and external stakeholders. Perform other duties as assigned. Qualifications/ Experience Requirements Bachelor's degree in social work or a related field or equivalent combination of education and experience is required. Experience in healthcare, social services, or homeless services preferred. Experience should be in vulnerable populations such as homelessness, mental illness and substance use disorders preferred. Valid driver's license with clean driving record required. Proficiency with Microsoft Office suite and products, data collection and reporting. Excellent verbal and written communication skills. Organizational and problem-solving abilities. Ability to work independently and make informed decisions in high pressure situations. Ability to exercise flexibility, creativity and good judgement. Must have appropriate handling of protected healthcare information. Supervision Exercised Exercises no supervision.
