This program serves individuals with mental health and substance use disorders. Hours:
40 hour work week: 8am-4:30pm (30 minute unpaid lunch) Sunday-Thursday (2 openings) Tuesday-Saturday (2 openings) Monday-Friday (3 openings)

Case Coordinator:
A high school diploma or equivalent Valid NYS Driver's License required Responsibilities for the Case Coordinator include : Responsible for a designated case load; including the accurate and timely completion of case record documentation in the Agency electronic health record. Establish and maintain good rapport and therapeutic relationship with residents. Aid and encourage clients to meet the established goals of their service plans. Teach, monitor and evaluate client's skills of daily living. Monitor and Supervise medications with clients. Meet with clients at least 5x per month and provide restorative services. Assist with transportation as needed. Hold a caseload of approximately 10-12 clients. Provide compassionate and person centered services. Weekly on-call rotation approximately once every three months.
