Responsibilities:
Job Responsibilities:
1. Case management responsibility for 10-15 young adults who are living in New York Foundling's
NY 15/15 & ESSHI
Vital Brookdale Programs 2. Adhere to Fair Futures Coaching model and provide Core Case Management services for each case, with the goal of supporting participants independent living skills and overall wellness. 3. Coordinate youth's service needs. Make appropriate referrals and follow-up with participant and service provider to ensure young adult is accessing services. Where youth is not accessing services, coach young adult and advocate on their behalf to help remove barriers. 4. Conduct required monthly in-office and home visits with assigned cases. Complete additional meetings as needed to ensure optimal outcomes for the case. 5. Provide flexible scheduling to complete case meetings and prioritize participant's schedule. 6. Collaborate with program vocational specialist to ensure that they are receiving appropriate vocational and educational referrals and skill building coaching. 7. Work with young adult to facilitate their integration into the community, support their ability to live independently, support the develop of relational skills to secure a broad support network. 8. Supervise or facilitate workshops, field trips, community days and enrichment activities for young adults in the program. 9. Attend trainings and conferences for professional development. 10. Maintain clear, concise and accurate documentation of progress that promote peer and supervisory review and feedback, and that demonstrates compliance with evidenced based clinical model and meeting our agency's expectation. Assure documentation (data entry) for contacts, incidents, and reports are submitted timely (as indicated by program). 11. Timely data entry of all services practices of the program in agency record and for reporting, and other system as provided.

Social Work, Sociology Degree, Physicology Degree A minimum of 1-2 years of experience working with youth in foster care, young adults with substance use and mental health issues; young adults experiencing chronic homelessness; Familiarity with the NYC child welfare system Must be an excellent communicator and have good interpersonal skills. Must be a good writer and have demonstrated ability to complete tasks in a timely manner. Ability and willingness to work some weekends and evenings is required including response to emergency calls. Demonstrated ability to identify problems, develop solutions and take the lead in solving problems. Must be self-directed, as well as a team player. Must have excellent organizational and multi-tasking skills. Working knowledge of Word, Excel and ability to work with database programs such as Access. Ability to work outside of regular business hours including evenings and weekend hours, as needed.
PREFERRED
Commitment to NY Foundling, OTDA, and ACS Mission and values Familiar with the political and cultural landscape of the community Understanding/working knowledge of youth and young adults-serving systems and agencies. Ability to work with young adults' boards and representatives. Self-starter and independent worker Excellent communication skills required. Ability and willingness to work some weekend and evenings. Experience with and willingness to work with culturally diverse population, computer literacy, able to research current information on medical, social, and legal issue.
Education Required:
Bachelor's Degree from a U.S. accredited educational institution or Credential Evaluation for a non-U.S. educational institution
