Job Description

Case manager needed for agency serving domestic violence survivors to foster independence, secure permanent housing and independence.
Key Responsibilities Assessment and Planning:
Evaluate client needs, develop individualized plans to help participant meet goals and secure housing
Coordination of Services:
Connect participants with healthcare providers, social services, employment and community resources
Monitoring and Follow-Up:
Track client progress, adjust plans as needed, and provide crisis intervention when necessary
Communication:
Maintain regular contact with clients, families, and service providers to ensure continuity of care
Documentation:
Maintain accurate records of client interactions, care plans, and outcomes
Empowerment:
Support clients in accessing benefits, completing forms for social programs.
Financial skills:
encourage participation in financial literacy classes, credit repair and programs that offer employment skills Required Skills and Attributes Strong interpersonal and communication skills for working with clients, families, and professionals Organizational and problem-solving abilities to manage multiple cases efficiently Knowledge of community resources, housing, programs, educational and employment opportunites Empathy, compassion, and critical thinking to address client needs effectively Proficiency in computer applications, including MS Office and case management software
Experience/knowledge of Trauma Informed Care Pay:
$55,000.00 per year
