Job Description

We are seeking a dedicated and compassionate Bilingual Spanish/English Supportive Housing Case Manager to join our team on a full time basis. The ideal candidate will have a high school diploma or GED, be available to work evenings and weekends as needed, and possess a valid driver's license and reliable transportation. This role requires knowledge of the mental health system and a comprehensive understanding of community resources that support residents in maintaining stable housing. Responsibilities (include but are not limited to):
  • Provide case management services to residents, including assessment, planning, advocacy, and coordination of services.
  • Develop individualized support plans in collaboration with residents to address their specific needs and goals.
  • Connect residents to appropriate community resources and services, including mental health support, healthcare, employment assistance, and educational programs.
  • Conduct regular home visits and follow-up meetings to monitor progress and provide ongoing support.
  • Maintain detailed and accurate records of resident interactions, services provided, and progress made.
  • Collaborate with other team members and external agencies to ensure comprehensive and coordinated support for residents.
  • Provide crisis intervention and support as needed.
  • Travel to various locations to meet with residents and attend meetings as required.
  • Conduct phone call screenings and other duties as assigned while working for a non-profit organization.
Requirements:
  • High school diploma or GED is required.
  • Valid driver's license and reliable transportation.
  • Availability to work evenings and weekends as needed.
  • Experience with the mental health system and knowledge of community resources.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a team.
  • Compassionate, patient, and understanding of the challenges faced by residents.
