Job Description
About HONOR:
- HONOR is a non-profit organization dedicated to providing support and services to homeless individuals and families. Our mission is to offer hope through hospitality by providing shelter, advocacy, and resources to those in need.
Position Summary:
- The Case Manager (CM) at HONOR plays a crucial role in providing support and assistance to homeless clients, including single men, single women, and families.
Monday, Wednesday, Friday:
8:00 AM- 4:00
PM, Tuesday:
10:00 AM- 6:00
PM, Thursday:
12:00 PM- 8:00
PM OR Monday, Tuesday, Friday:
8:00 AM- 4:00
PM, Wednesday:
12:00 PM- 8:00
PM, Thursday:
10:00 AM- 6:00 PM
Key Responsibilities:
- Provide individualized case management services to homeless clients, including assessment, goal setting, and referral to appropriate resources.
- Conduct crisis interventions and provide support to clients in need.
- Collaborate with coworkers, supervisors, and staff from other departments to ensure client needs are met and to improve processes and services.
- Maintain accurate and up-to-date client records and documentation.
- Participate in team meetings, trainings, and professional development activities as required.
- Maintain confidentiality and adhere to all ethical and professional standards.
Qualifications:
- Bachelor's degree in social work, Psychology, or a related field.
- 1-3 years of experience in the human services field preferred, but not required.
- Ability to relate to and work with individuals from diverse backgrounds, including those who may be ill, disabled, or emotionally distressed.
- Integrity, positive attitude, and self-directed with a passion for HONOR's mission.
- Must have and maintain a valid New York State driver's license.
- Detail-oriented with strong communication and interpersonal skills.
- Bilingual skills preferred, but not required.
- Experience with crisis interventions and individual/small group counseling is a plus.
