The Community Housing Case Manager provides individualized, field-based case management and supportive services to families and adults enrolled in HONOR's Community Housing Permanent Supportive Housing (PSH) program in Orange County. Participants reside in scattered-site apartments master leased by HONOR. The position helps participants maintain permanent housing, strengthen independent living skills, connect with treatment and community resources, and improve income and self-sufficiency. Working with participants, the Case Manager assesses needs, develops housing and service plans, coordinates services, and monitors progress. The position collaborates with program leadership, community providers, and HONOR's facilities team to address barriers to housing stability and fulfill applicable U.S. Department of Housing and Urban Development (HUD) Continuum of Care (CoC) requirements.
QUALIFICATIONS FOR THE POSITION
1. Bachelor's degree in a human services or related field required; two to three years of relevant human services experience preferred. 2. Experience supporting individuals and families experiencing homelessness, disabilities, mental health conditions, or substance use disorders preferred. 3. Strong assessment, organization, documentation, communication, and interpersonal skills; ability to manage a caseload, meet deadlines, and work independently and as part of a team. 4. Ability to engage participants respectfully, respond calmly to conflict or distress, and work effectively with people from varied backgrounds. 5. Proficiency with electronic records and common office software; ability to learn HMIS and applicable HUD program requirements. 6. Valid New York State driver's license and ability to meet agency driving requirements. 7. Complete SOAR certification within 90 days of hire. 8. Bilingual skills preferred.

$24.00 per hour
