$27.47 per hour
Hours:
Monday-Friday 9:00AM-5:00PM (35hrs)
PERCENT OF TRAVEL
This job can require up to or more than 10 hours per week on a full-time basis, depending on the need of the clients, in which is equivalent to 30% travel expectations per week
OVERVIEW OF PRIMARY RESPONSIBILITIES
Provide case management services to assigned participants Function as a team member through attendance of monthly staff meetings Conduct home/office visits weekly or as required. Assist participants in applying for Section 8 certificate or other appropriate Housing referrals. Assist and accompany participants to DSS & Social Security Administration for entitlements, as needed to apply for and secure benefits. Provide referrals and linkage of participants as needed to substance use treatment, vocational training, and various community resources and conduct appropriate follow-up. I.e. Health Home and Care Coordination providers. Provide assistance/referrals and linkage of participants to employment and educational opportunities. Evaluate apartment's conformance to Housing Quality Standards for safety and habitability. Maintain assigned charts Progress notes (Weekly) Service Plans (Updated every three months) Participants consent forms (Yearly or as needed), and Other required documentation (screening form, enrollment form, occupancy agreement rent calculation, and etc.) Provide supportive counseling to participants on caseload. Work in tandem with Peer Advocate on assigned tasks. Complete required program report (monthly and quarterly) Act a liaison with other service providers to help ensure appropriate service delivery. Facilitate and mediate communication with assigned participants and landlords as needed. Participate in case management and professional development activities such as trainings webinars, and continuing education services. Complete all other assignments.
QUALIFICATIONS AND ATTRIBUTES
Valid & Unrestricted Driver's License Work History with Diverse population
CASAC, CASAC-T
preferred.
EDUCATION AND EXPERIENCE
Minimum of a
AS, BA/BS
in related field Minimum of 2 years working in a Mental Health/Substance Abuse Field
