Case Coordinator

Position works within a community residence and community apartment program providing services to individuals with mental illness. Responsibilities include overall care of the residents, upkeep of the house/apartment, ADL training, service planning, daily notes related to progress, transportation, participation in recreational activities, advocacy, counseling, and crisis intervention. High School Diploma or GED, and valid NYS driver's license required.
