Job Description

Case Manager - Enhanced Young Adults Program The Case Manager is responsible for coordinating case management services, linking clients to community-based resources, and providing the necessary education and support to help young adults maintain stable housing. The role involves developing individualized support plans and fostering connections to essential services. Duties include 1. Assess clients' needs and strengths for services in the community and eligibility for all funding sources. Connect clients to all appropriate funding sources upon admission to program. Coordinate completion of entitlement forms and submit within time deadline. 2. Identify any immediate crisis intervention needs and services. Develop a support plan. 3. Provide referral and linkage to community services as needed. 4. Assess, refer, and provide information on basic needs. Assess the transportation needs of the client and provide referral and education regarding available transportation. 5. With clients, develop a schedule of visits and provide continual adjustment to the number of visits based on the clients' needs. 6. Provide education and support in daily living skills including medication, health, and safety. 7. Provide on-call crisis intervention services on a rotating schedule amongst team members. 8. Maintain an Electronic Case Record for each client on the assigned caseload. 9. Provide transportation to clients using the agency's vehicles. 10. Promote a culture of inclusion and belonging. Requirements Skills/Qualifications for the Case Manager include: 1. Bachelor's degree in human services preferred. Experience in human service and/or college classes will be considered. 2. Two years working in human services including experience working with young adults. 3. One year's experience in case management, 4. Valid and insurable driver's license. Agency vehicles are available for transporting clients. Must have a personal vehicle to use for business purposes when not transporting clients. 5. Ability to organize and have clear communication both orally and in writing, 6. Excellent organizational skills, problem-solving skills, ability to be flexible, and the ability to follow through while multiple tasking. Benefits Salary and Schedule of the
Case Manager:
