Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings. About the Role We are looking for a proactive employee to provide the necessary supports to eligible persons that promote success at the highest level of independence possible.
Responsibilities include:
o Develop supportive relationships with individuals and get to know their unique capabilities, personality, and potential o Support individuals by providing assistance with daily living skills and oversight, as needed o Assist individuals with seeking out services, promoting choice, and assisting in their understanding of rights and responsibilities o Recognize, encourage, and support, opportunities that promote natural connections within each participant's community o Maintain a person-centered approach and support individual preference and choice to the extent possible under program guidelines o Take an active advocacy stance for participants while teaching them to advocate for themselves whenever possible to facilitate skill development o Possess a broad knowledge of community services which can be made available to participants, family members, service providers, and other significant support people o Maintain ongoing relationship with community residence staff as assigned to facilitate discharge planning o Complete all program documentation in a timely manner and per Agency standards; documentation includes, but is not limited to: service plans, service notes, financial, and program outcomes, etc. o Conduct self in a professional manner o Participate in Agency-wide events, advocacy, and/or community education o Provide person-centered, quality services o Observe confidentiality and safeguards all participant related information o Follow Agency standards and procedures o Ensure compliance with federal, state, and local employment laws and regulations, as well as company policies o Perform Other Duties as Assigned Education & Experience Associate degree, plus 3 years' human services experience, required. Bachelor degree, preferred. Will consider equivalent combination of relevant education and experience. Conditions of Employment Must have a valid NYS Driver License in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
