Paid by employer Full job description The Case Worker in Adult Protective Services (APS) protects vulnerable adults experiencing abuse, neglect, or exploitation. You'll assess risks and needs, develop individualized service plans, conduct home visits, coordinate services with community providers, and document progress to support client safety, stability, and independence. Duties include formulating plans, providing counseling, coordinating with other providers, making home visits, and maintaining case records. Responsibilities Conduct risk/needs assessments and formulate and carry out plans for individuals and families. Provide supportive counseling; make home visits; coordinate with social service, health, and justice partners. Maintain accurate, confidential records; review cases periodically and update plans. Minimum qualifications Bachelor's degree from a regionally accredited college or university.

Valid New York State driver's license and access to a vehicle. Preferred skills Empathy, respect for individual differences, and strong observation/analysis. Clear written and verbal communication; ability to collaborate across agencies.
