They conduct comprehensive interviews with individuals dealing with personal and family adjustments, potential foster care situations, financial struggles, employment issues, and various other concerns related to food, clothing, housing, and physical or mental impairments. Through these interviews, the Case Worker gathers important information about the clients' medical, psychological, and social factors, enabling them to assess the nature and severity of the problems. They then offer counseling services on an individual or group basis, assisting clients in developing plans to address their needs and empowering them to tap into their inner strengths and external resources for improved social functioning. The Case Worker regularly meets with clients as required by funding sources, monitoring their progress and providing guidance to modify attitudes and behaviors for positive change. Additionally, they connect clients with community resources and organizations, establishing a network of support. Administrative tasks such as completing necessary reporting forms, maintaining accurate client case files, and ensuring effective communication with supervisors and funding sources are also part of their responsibilities. The Case Worker remains vigilant about client safety, promptly notifying relevant parties of any potential safety issues. This position is Full Time at 35 hours a week. Entry:

Associate Degree in Human Service or equilvvent. A combination of education and experience can be considered At least three years of experience working with vulnerable individuals. Candidates with lived experience are preferred and encouraged to apply.

Valid N.Y.S. Driver's Registered and Insured Vehicle N.Y.S.

Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities that may not meet Federal accessibility standards for disabled individuals. Manual dexterity sufficient to work with children/adults in a challenging and active environment.
