The Social Care Screener supports the Social Care Network by helping identify unmet social needs among Medicaid members. This role focuses on conducting screenings, gathering information, and connecting members with appropriate team members for service linkage. This position is ideal for someone seeking seasonal or summer employment and is interested in gaining experience in community health or human services.Responsibilities:
Conduct health related social needs screenings with Medicaid members by phone, virtually, or in person.
Collect and document member information in the Unite Us electronic record system.
Help connect members with appropriate staff or care team representatives for additional support and services.
Provide general information about available community programs and resources.
Maintain accurate and timely documentation.
Participate in required trainings and team meetings.
Follow agency policies and maintain professionalism when working with members and community partners.
Perform other related duties as assigned.Competencies:

Basic organizational and time management skills. Comfortable using computers and electronic record systems.

Key Performance Indicators (KPIs): Meets productivity expectations for clients screened.

Completes and maintains all required trainings under the SCN model.
Positive client outcomes and satisfaction.Requirements:
High school diploma or equivalent required.
Associate degree or coursework in human services, social work, psychology, healthcare, or related field preferred.
Experience working with community members, customer service, healthcare, or social services is a plus.
Basic computer knowledge: typing, communication tools, scheduling tools and web browsers.
Proficiency in the Microsoft Office suite and database management systems. Fluency in English (Reading, Writing, Speaking).Must be and maintain clearance by applicable regulatory clearances as required.
