Job Description

As the Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through: Completes the timely and accurate documentation in the Uniform Case Record, including but not limited to: referral intake packets and related intake information, referrals and related correspondences, entitlement documentation, logbooks, and other areas as assigned. Reviews all intake information for new referrals and performs initial screening assessments for intake criteria and makes recommendations regarding acceptance or denial. Perform Catholic Charities Housing Office intake assessments of each new tenant and then annually thereafter. Develops and implements the ISP and then every 6 months thereafter, reports outcomes and makes needed changes. Organizes and facilitates support groups and social and recreational activities including but not limited to issues relating to daily living skills, treatment, housing permanency concerns, and entitlement eligibility. May perform the intake and orientation of new referrals to the program during periods of high activity as needed. Participates as a team member in the development of the program routine for the residents based on consumer feedback and discussion at regular staff meetings. Processes statistical information, progress reports, and written discharge information, using computer based software. Compiles and submits data to the Director of Support Services, for the preparation of monthly, quarterly, and annual reports.

$ 20.00/hour
Case Manager Maximum Salary:
$ 23.00/hour
Case Manager Schedule:
Wednesday•Saturday, 10:00am•8:00pm (40 hours/week)
Qualifications:
