Job Description
Case Manager Cedar Ridge Behavioral Health Solutions Cambridge, OH Full-Time | Monday-Friday | 8:00 AM - 5:00 PM Join Our Team Cedar Ridge Behavioral Health Solutions is seeking a dedicated and compassionate Case Manager to join our team in the Cambridge office . This position plays an important role in helping individuals access resources, build life skills, and work toward recovery and stability. The Case Manager will work closely with clients receiving mental health and/or substance use treatment, helping them navigate services and connect with community supports that promote long-term success. What You'll Do Provide case management services to individuals receiving behavioral health and substance use treatment. Assist clients in identifying personal goals and developing individualized service plans. Connect clients with community resources such as housing, employment opportunities, medical services, and social supports. Coordinate care with therapists, medical providers, and other members of the treatment team. Maintain regular contact with clients through scheduled meetings and phone calls. Monitor progress and help clients overcome barriers to achieving their goals. Complete timely and accurate documentation in the electronic health record. Participate in team meetings, case consultations, and supervision. Qualifications Bachelor's degree in Social Work, Counseling, Psychology, Human Services, or a related field preferred , OR Associate degree with relevant behavioral health experience , OR High School Diploma/GED with relevant experience working in behavioral health, social services, or related field will be considered Ideal Candidate Passionate about helping individuals in recovery Strong communication and organizational skills Ability to work both independently and as part of a team Comfortable working with community resources and service providers Basic computer and documentation skills Schedule Full-Time Monday - Friday 8:00 AM - 5:00 PM No weekends required About Cedar Ridge Behavioral Health Solutions Cedar Ridge Behavioral Health Solutions is committed to providing high-quality mental health and substance use treatment services.
