Lead Case Manager Are you committed to supporting children and families through life-changing, community-based care? Lead Case Manager Responsibilities include but are not limited to: Travel to provide in-home and community-based services to youth and families Manage a caseload of foster youth and support foster homes Coordinate services with schools, courts, medical providers, and community partners Conduct assessments and assist in developing individualized service and treatment plans Support family reunification and placement stability efforts Maintain timely and accurate documentation of visits, services, and case progress Build and maintain strong relationships with youth, families, and stakeholders Participate in team meetings, court involvement, and on-call rotation as needed Lead Case Manager Qualifications include but are not limited to: Bachelor's Degree in Social Work, Counseling, Psychology, Sociology, or related human services field LSW or LPC is required Minimum 2 years of relevant experience preferred (child welfare or working with youth strongly preferred) Strong communication, organizational, and problem-solving skills Ability to work independently and manage a flexible schedule Valid driver's license, reliable transportation, and ability to travel daily Lead Case Manager Benefits include but are not limited to: Strong base salary!
