Job Description

Waiver Coordinator Location:
Empowering People Inc., 840 North Market Street, Lisbon, Ohio 44432
Duties:
Primary responsibilities include hiring, training, scheduling, supervising, discipline, evaluation and managing Direct Support Professionals in the implementation of services and programs. Ensuring DSP's are meeting health and welfare needs of the individuals they serve, Will assess, develop, implement, and review plans to assure services are provided according to the individual service plan and in compliance with local, State, and Federal guidelines. Encourages and enhances the involvement of guardians, family, community, and other service providers. Review utilization and ensure cost-effective services for all individuals to ensure budget expectations are met. Assist in crisis intervention. Act as a team member for individuals and participate in the development of individual service plans (ISP). Advocate for individuals and support them in participating as fully as possible in the communities in which they live. Document all services being performed and assure Direct Support Professionals document according to departmental rules and guidelines. Transport individuals to and from appointments, meetings, and shopping as needed. Audit and review consumer financial records monthly. Assist in obtaining and maintaining benefits for individuals served. Make referrals for needed services for individuals as assigned. Must inform Director of all incidents, MUI's, or other problems that arise in the homes served. Must monitor all shifts at each home served on a regular basis. Demonstrates knowledge of and implements all internal policies and procedures. Follows all state and federal rules, laws, and regulations pertaining to operation of community/waiver settings. Maintains effective communication with families/guardians, co-workers and outside agencies. Promotes a positive, cooperative relationship with families/guardians, co-workers and outside agencies. Develops and maintains a professional relationship with employees. Promotes positive morale, mutual respect, and an environment of teamwork and cooperation. Complies with emergency procedure requirements. Attends and participates in meetings and trainings as required. Performs duties in a manner that respects the resident's rights and maintains confidentiality. Participates in quality assurance reviews. Assists in maintaining a safe, clean, and attractive home environment. Supports the mission, visions and values of the organization. Performs all other duties as assigned by the Director.
Minimum Requirements:
Bachelor's degree in human services or related field or equivalent experience preferred Minimum one year experience in working with individuals with Developmental Disabilities required. Current, valid Ohio driver's license and personal car insurance at facility minimum levels documentation required. Must maintain CPR, First Aid, and Medication Administration certifications, Must have the ability to provide therapeutic interventions during crisis situations and effectively communicate with both verbal and non-verbal individuals. Ability to use discretion and independent judgment in oversight responsibilities and the delivery of services provided. Must successfully complete orientation training and background checks.
Hours Worked:
Hourly- Full time position; May need to work beyond normal working hours, on weekend and holidays and on other shifts when necessary.
