Case Manager TASC of Northwest Ohio

Responsibilities:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop appropriate service plans
- Collaborate with community resources to ensure clients receive necessary support and referrals
- Monitor client progress and make adjustments to service plans as needed
- Advocate for clients' rights and access to services
- Provide crisis intervention and support during emergencies or high-stress situations
- Assist clients in accessing housing resources, including Section 8 assistance
- Provide addiction counseling and support for individuals struggling with substance abuse
- Mentor clients and provide guidance on life skills development
- Coordinate with behavioral health professionals to address mental health needs
- Familiarity with Medicaid regulations and processes
- Knowledge of child welfare systems and ability to navigate related services
- Utilize motivational interviewing techniques to promote behavior change
Skills:
- Strong crisis management skills to effectively handle emergency situations
- Proficient in crisis intervention techniques to provide immediate support and stabilization
- Knowledge of social work principles and practices to guide client interactions
- Understanding of Section 8 housing programs and eligibility requirements
- Experience in addiction counseling and knowledge of evidence-based treatment approaches
- Ability to mentor individuals and provide guidance on personal growth and development
- Familiarity with behavioral health services and ability to collaborate with professionals in the field
- Understanding of Medicare regulations and processes for assisting clients with healthcare access
- Knowledge of child welfare systems, policies, and procedures for working with families in need
