Job Description

The Bridges Liaison will provide direct services such as housing support, case management and independent living skills to all assigned young adults who age out of foster care in Ohio and are eligible for Bridges.
Responsiblities:
Assist the young adults in developing a plan and goals Have face to face contact at least once a month Review the plan every ninety days to acknowledge achievements and plan your next steps Assist young adults to maintain eligibility if their circumstances change Identify individuals and resources that can be permanent connections Teach skills like money management, how to do laundry, how to grocery shop, etc. Assist young adults if they choose to enroll in college or a vocational program Assist young adults in finding employment Help young adults locate a doctor or access other physical or mental health care Connect to community resources to help you become more self-sufficient Provide guidance and be a positive young adult supporter Other duties as assigned
Education/Experience/Qualificaitons:
Bachelor's degree in social work, human services, behavioral health, or a related field is required. Two years of experience working with families and/or children in the child welfare system is preferred. Must be 21 years of age or older.
