$50,000-$55,000 About Us At the Area Office on Aging of Northwestern Ohio (AOoA), we are committed to helping veterans, older adults, individuals with disabilities, and caregivers remain healthy, independent, and supported in their homes and communities. Position Summary The VA Self-Directed Care Person-Centered Counselor conducts assessments, coordinates services, and supports veterans participating in the VA Self-Directed Care Program. This role ensures compliance with program requirements while promoting person-centered care and independence. Key Responsibilities Conduct initial and annual assessments to evaluate needs and service gaps Assist with enrollment documentation and program onboarding Develop and monitor care plans and spending plans Coordinate with VA and Financial Management Services (FMS) Provide education on program requirements and employer responsibilities Monitor participant health, safety, and service delivery Maintain accurate and timely documentation (within 24 hours) Submit required monthly reports Support participant rights, choice, and self-determination Travel throughout the service area as needed Perform other duties as assigned Qualifications Licensed Social Worker (LSW) or Registered Nurse (RN) in Ohio (active and unencumbered) At least one (1) year of experience in home health, case management, social work, behavioral health, or related field Preferred Experience conducting health and social assessments Knowledge of community resources, family dynamics, and human behavior Strong communication, organization, and problem-solving skills Proficiency in Microsoft Word and Excel Ability to work independently and as part of a team Schedule & Work Environment Four-day work week (Monday-Thursday or Tuesday-Friday) Based in Toledo, Ohio, with travel required Additional Expectations Maintain HIPAA compliance and confidentiality Participate in required trainings Demonstrate cultural competency Adhere to agency policies and procedures Physical Requirements Lift/carry up to 20 pounds Valid driver's license, reliable transportation, and insurance required Special Requirements Employment is conditional upon successful completion of a background check, drug/alcohol screening, and Motor Vehicle Record (MVR) check. Screenings may be repeated per agency policy. Cross-training may be required. This position is contingent upon funding. Why Join AOoA?
