Case Manager 3.1 3.1 out of 5 stars Warren, OH $19 an hour - Full-time Coleman Health Services 209 reviews $19 an hour - Full-time Seeking a full time Case Manager to provide case management services to individuals with severe and persistent mental health issues who may also have a substance abuse disorder in Trumbull County.

Transition clients and families into ongoing care Work collaboratively with treatment teams internally and externally to ensure safe transitions Qualifications of the Case Manager Bachelor's degree in a social service-related field or three-years' experience Willing to get NPI and Medicaid Billing number (we'll help!)
