In this vital role, you will coordinate patient care plans, facilitate communication among multidisciplinary teams, and ensure seamless discharge planning to promote optimal health outcomes. Your expertise will help navigate complex healthcare systems, support patients through their recovery journey, and uphold the highest standards of patient service. This position offers an exciting opportunity to make a meaningful difference in patients' lives while working in a fast-paced, collaborative environment. Responsibilities Assess patient needs through comprehensive intake and evaluation processes, utilizing medical terminology and EMR (Electronic Medical Record) systems. Develop individualized care plans that incorporate critical care, hospice care, behavioral health, and pediatric considerations as appropriate. Coordinate with hospital staff across units such as
ICU, PICU
(Pediatric Intensive Care Unit), trauma centers, and emergency medicine to facilitate timely interventions. Conduct utilization review and management to ensure appropriate level of care while adhering to HIPAA regulations and managed care protocols. Collaborate with social workers, physicians, nurses, and other healthcare professionals to optimize discharge planning and patient transitions across settings. Perform case management activities including patient advocacy, discharge coordination, and follow-up to support ongoing recovery and community integration. Maintain accurate documentation of case progress, ICD coding (including
ICD-9/10
), CPT coding, and medical records to support billing and compliance requirements. Qualifications Proven experience in hospital settings with exposure to acute care, ICU, Level I or Level II trauma centers preferred. Strong background in nursing or social work with knowledge of medical terminology and EMR/EHR systems. Familiarity with ICD coding (ICD-9/10), CPT coding, utilization review, and discharge planning processes. Experience working within managed care environments or utilization management is highly desirable. Excellent communication skills with the ability to collaborate effectively across multidisciplinary teams. Ability to handle complex cases involving behavioral health, pediatrics, or hospice care with sensitivity and professionalism. Knowledge of HIPAA regulations ensuring patient confidentiality at all times.
