Position Summary The Case Manager is responsible for clinic-wide care coordination across both Rehabilitation Services and Applied Behavior Analysis (ABA) programs. This role ensures continuity of care, removes barriers to treatment, and supports families in maintaining consistent engagement in services. Serving as a central point of coordination, the Case Manager works directly with families, clinical teams, and community resources to proactively address factors that impact attendance, documentation compliance, and access to care. This role is critical in reducing service disruptions, improving attendance, and supporting overall clinic productivity. Key Responsibilities 1. Care Coordination & Family Engagement Conduct proactive outreach to families following: Missed evaluations Missed appointments Patterns of poor attendance Provide support, education, and problem-solving to prevent continued service disruption Maintain consistent communication with families across both ABA and Rehab services 2. Continuity of Care & Documentation Management Track and monitor all required documentation to prevent lapses in services, including: Annual physician orders/scripts for therapy services Assist families with: Scheduling well-child checks or required physician visits Coordinating follow-up to obtain updated documentation Ensure timely receipt and submission of all required documentation to maintain active services 3. Barrier Identification & Resolution Identify and address barriers impacting service participation, including: Transportation challenges Food insecurity Scheduling conflicts Family resource limitations Develop individualized support plans to improve attendance and engagement 4. Resource Navigation & Community Connections Connect families with community resources such as: Transportation assistance Food support programs Financial or social services Maintain and regularly update a resource database for local and state services Follow up to ensure successful connection and utilization of resources 5. Insurance & Eligibility Support Assist families with: Insurance enrollment and re-enrollment Resolving coverage issues that may delay or interrupt services Support completion and submission of required documentation for: Insurance authorization Eligibility for ABA and Rehab services Coordinate with internal teams to ensure compliance and timely processing 6. Referral & Diagnostic Support Assist families in obtaining necessary referrals, including: Developmental pediatricians Diagnostic providers for Autism evaluations Guide families through the process of securing an Autism diagnosis for ABA eligibility when applicable 7. School Collaboration & Advocacy Support Connect families with advocacy resources for: IEP services School-based support systems Encourage coordination between clinic services and educational settings 8. Early Risk Identification & Team Collaboration Collaborate with: Front office staff BCBAs Therapists and clinical teams •
To:
Identify at-risk families early (attendance concerns, documentation gaps, disengagement) Develop and implement proactive intervention strategies Communicate updates and barriers impacting care across departments Performance Expectations / Success Metrics Reduction in missed appointments, cancellations, and no-shows Improved attendance and consistency across ABA and Rehab services Decreased lapses in care due to documentation or insurance issues Increased successful connection of families to community resources Timely completion and maintenance of required documentation Minimum Qualifications (Required) Bachelor's degree in Social Work, Healthcare Administration, Psychology, Human Services, or related field Minimum two (2) years of experience in case management, care coordination, social services, or a similar role Strong organizational, problem-solving, and follow-through skills Excellent communication and interpersonal abilities Bilingual Preferred Qualifications Experience in pediatrics, ABA services, pediatric rehabilitation therapy, or behavioral health settings Knowledge of Medicaid, insurance processes, and healthcare systems Experience working with underserved or high-need populations Key Competencies Strong critical thinking and problem-solving High level of empathy and family-centered approach Ability to manage multiple priorities across departments Systems navigation (medical, insurance, community resources) Accountability and consistent follow-through Work Environment Clinic-based role supporting both ABA and Rehabilitation departments Frequent communication with families (phone, email, in-person) Ongoing collaboration with clinical, administrative, and leadership teams Regular coordination with external providers and community agencies Position Impact This role directly supports:
