Tulsa, OK Job Details $42,000 a year 19 hours ago Qualifications High school diploma or GED Driver's License Productivity software Full Job Description The Case Manager for A Better Way Program will be responsible for managing the caseload of service recipients participating in the A Better Way program. This will require intensive and integrated outreach and case management targeting individuals experiencing challenges related to homelessness, unemployment, health concerns, substance use and mental health needs. The Case Manager will be expected to utilize strength-based engagement and empowerment approaches to establish long-term relationships built on trust and support for all program participants. This position works in conjunction with the Employment Specialist in the A Better Way Program as well as the Van Driver.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Comprehensively assess participant's needs and work closely with these individuals to develop, coordinate, implement, and monitor service plans with the aim of achieving their identified goals. As program participants near completion of the program, the Case Manager will maintain a supportive, persistent, and reliable relationship to ensure self-sufficiency is achieved, utilizing best practice graduation methods. Work with supervisors and community partners to support individualized treatment plans for each client that include all activities and services that are essential to meet the specific needs of the client. Maintain a caseload of clients, monitor client progress by regular, ongoing contact with individuals receiving services and contact with collaborative service providers both external and internal and external agencies. Perform other duties as assigned.
KNOWLEDGE, SKILLS & ABILITIES
Education:
High School Diploma or GED required. Bachelor's degree in social service/human services preferred or a relevant combination of education and experience.
Experience:
Experience working with persons with mental illness preferred.
Skills:
Excellent organization and time management skills. Must possess working knowledge of technology including Microsoft suite programs Ability to communicate clearly and effectively, both verbally and in writing. Ability to work closely with a multidisciplinary team that includes various partner agencies.
Certification:
Current Oklahoma License in one of the following preferred: Social Work or Case Management. Must be eligible for Case Manager certification through
ODMHSAS. WORK CONDITIONS & PHYSICAL DEMANDS
Job will be performed in various settings including office, community, and outside environmental conditions. Noise level is moderate and includes sounds of normal office equipment (printers, fax machine, phones, etc.) as well as verbal communications with individuals we serve. Required to work in areas inhabited by homeless people, to include working in dirty environments and dealing with challenging individuals. Potential exposure to stressful and/or physically challenging situations that could result in risk to one's safety. Work requires a flexible work schedule due to internal and external meetings. Occasional travel may be required; you must have reliable transportation and a valid Driver's license. Ability to effectively communicate orally and in writing. Physical ability to lift and carry 50 pounds. Visual and auditory ability to identify and respond to environmental and other hazards of the site and staff behavior. Physical mobility and endurance to perform tasks while standing/walking for long periods of time (60 minutes or more). Ability to safely and properly use office equipment.
