Overview Join our dedicated team as a Case Manager specializing in Supportive Services for Veteran Families (SSVF). In this vital role, you will empower individuals experiencing homelessness or at imminent risk by providing comprehensive case management, resource navigation, and supportive interventions. Your energetic approach will help clients stabilize their housing, develop life skills, and access essential services, fostering independence and resilience. This position offers an exciting opportunity to make a meaningful difference in the lives of vulnerable populations through proactive engagement and personalized support. Housing Navigation Conduct housing-related intake and assessments. Research and build relationships with landlords and housing providers. Assist clients with housing searches and accompany them to view units. Advocate for clients during the application and leasing process. Provide tenancy support and conflict resolution post-placement. Case Management Deliver weekly case management and support services until clients are stabilized. Provide aftercare for up to three months post-housing placement. Connect clients with resources for basic needs, healthcare, employment, and more. Submit documentation for financial assistance (rent, utilities, deposits). Attend team meetings and complete required documentation. Qualifications Required Skills & Knowledge Strong understanding of housing laws, tenant rights, and local housing markets. Ability to build rapport with diverse populations and prioritize client needs. Familiarity with community resources, mental health, and substance abuse services. Experience working with homeless or at-risk populations; veterans preferred. Excellent communication and documentation skills. Proficiency in computer systems and case management tools. Education & Experience High School Diploma or equivalent. Minimum of 2 years of professional experience in a related field. Why Join Our Talent Pool?
