Job Description

Supportive Services Navigator - Lakeview, Oregon Job Summary Join our dedicated team as a Supportive Services Navigator, where you will play a vital role in connecting individuals and families to essential resources and services that promote stability, independence, and well-being. This position offers an exciting opportunity to make a meaningful impact by providing personalized guidance, crisis intervention, and advocacy within diverse communities.
Definition:
Under supervision of the Lakeview Property Manager, has responsibility of providing supportive services to KHA Permanent Supportive Housing residents to ensure their housing success.
Essential Duties:
Comply with all approved KHA policies and procedures, federal regulations and state statues as they relate to the various programs; Provide direct service to participants, including case management; Assist participants in becoming self-sufficient and overcome barriers to maintain permanent housing; Develop action plans that identify participant needs, barriers, and strengths; Participate in regular follow-up meetings and check-in with clients; Case note tenants' progress; Develop and maintain strong relations with other agency partners; Schedule community events; Conduct meetings and group presentations; Assess clients' eligibility and aid in gathering documentation as required for intake, enrollment and program participation; Assist tenants with transportation if needed; Advise tenants of program regulations; Establish and maintain accurate computer records of all tenants on caseload; Keep onsite office and common areas, including laundry and bathroom, clean and orderly; Perform move-in orientation; Perform other related duties as assigned
Behavioral Requirements:
Provide backup and support to co-workers; Establish and maintain professional and effective working relationships with tenants, co- workers and other agencies; Maintain punctual and regular attendance; Keep workspace, materials and common areas neat, clean and organized; Comply with department and Housing Authority policies, procedures and regulations; Participate fully as a member of the team; Participate in employee training and orientation; Provide training to co-workers as requested; Perform other job duties as assigned by supervisor.
Physical Requirements:
Operate a motor vehicle in the performance of duties; Maintain a current Oregon driver's license and an acceptable driving record; Use depth perception; Operate a keyboard; Lift and carry up to 15 lbs.; Employ repetitive motion of hands and wrists; Read an 8 pt. font; Sit for extended periods of time; Stand, walk and traverse uneven terrain; Speak with a clear and audible voice; Hear a normal speech level.
Special Requirements:
Reasonable knowledge of the community and social and economic resources available to low-income and elderly individuals; Ability to form and maintain effective relationships with families, to analyze and interpret a family's housing situation; Clearly and concisely express self orally and in writing; Speak, read, write and understand English; Work harmoniously with other employees; Maintain a professional attitude and respect the confidentiality of sensitive information; Ability to remain calm, objective and understanding under angry and/or verbally abusive, hostile situations; Use independent action; Apply creativity; Employ appropriate proactive customer service skills; Make decisions; Use discretion; Exercise interpersonal skills; Utilize basic computer skills; and Employ deductive, inductive and cognitive reasoning. •The omission of specific statements of duties does not exclude them from the position.
