GENERAL PURPOSE
The Aging Case Aide 2 - Protective Services position is an advanced public contact work in the Area Agency on Aging (AAA) department providing information and referrals to Older Adults to assist them in obtaining a wide range of services.
SUPERVISION RECEIVED
This position reports directly to the Aging Care Management PS Supervisor.
SUPERVISION EXERCISED
This position does not exercise supervision over other functions.
ESSENTIAL DUTIES OF THE POSITION
Attends supervisory conferences/Unit meetings as scheduled. Attends training, locally or out-of-town, as approved by the Supervisor. Cooperates with co-workers and staff of other Division of the Department of Human Services and community agencies in the provision of service to shared consumers when appropriate. Serve as a contact for banks:
  • Develop relationships with key personnel at financial institutions.
  • Assist with gathering financial information on financial exploitation cases and obtain financial statements when indicated.
  • Accompany consumers to bank if necessary to receive bank records.
Obtain PB22 and Mandatory Act 13 incident Reports on Reports of Need (RONs) received from facilities (nursing homes, assisted livings, personal care homes, and adult day care centers). Do home visits as directed to assist with various errands such as picking up paperwork, dropping off food, etc. Keep resource information current. Provide assistance to Protective Services staff including:
  • Assistance with taking RONs for Protective Services, processing and tracking assignment of RONs, and completing required notifications.
  • Entering RONs and/or progress notes for PS Care Managers and/or PS Supervisors when needed.
  • Providing telephone contact support to implement and/or ensure follow through with designated areas of consumers' care plans as directed by PS Unit Director or PS Supervisors.
This includes such things as:
a) Follow-up with physicians to obtain reports and/or schedule competency evaluations. b) Order necessary formal/contract services and process service notifications in SAMS. c) Reestablish support networks for isolated consumers. d) Assist with the relocation process, i.e., complete housing or NH applications, and follow-up with facilities, etc., as well as arrange emergency shelter in interim. e) Assist PS Aging Care Managers with gathering other necessary information. Fax and e-mail information as directed. Checking Fax Folders and distributing faxes. Document all case activity in SAMS PS Care Plan Journal. Do steps necessary to close the consumer to PS as needed. File. Assist with running reports in SAMS, such as RON and PSI missing data reports. Delete and shred files in accordance with PS regulations. Contributes to a positive work environment.

REQUIRED MINIMUM QUALIFICATIONS EDUCATION & EXPERIENCE
 - One (1) year as a Case Aide 1 (Local government); OR Two (2) years of public contact work, including one (1) year in a human services program; OR Any equivalent combination of experience and training. Employee assigned to this title will be required to possess and maintain a valid and current motor vehicle operator's license. Must have an acceptable driving record. Personal vehicle required for County business, employee is eligible for mileage reimbursement.
KNOWLEDGE, SKILLS, AND ABILITIES
Knowledge of the physical, cultural, and social aspects of the assigned community. Ability to obtain, evaluate, and analyze information from interviews, reports, records, and other type of sources of information, and to provide information to clients and other types of personnel. Ability to establish and maintain positive working relationships. Ability to learn about Area Agency on Aging Programs and the services of other community agencies. Ability to communicate verbally with older persons. Ability to perform simple arithmetic computations. Ability to follow simple oral and written instructions. Ability to express ideas clearly and persuasively to elderly clients. Ability to clearly express ideas verbally, and in writing. Ability to maintain confidentiality at all times. Ability to communicate fluently in English, including telephone contacts, face-to-face contacts, and communications in large and small group settings. Ability to read the English writing in cursive and printed forms. Ability to analyze and interpret information that is received in written and verbal form. Travels to and interacts with consumers, members of the community, and representatives of other social agencies in their homes, offices, and other community locations. Attends training as required either locally or out of town.
TOOLS AND EQUIPMENT
Telephone, personal computer (including word processing and spreadsheet software), calculator, writing implements, fax machine, copy machine, and paper shredder.
