Supports Coordination is the service of developing, identifying, coordinating, monitoring, and maintaining supports and services for individuals with intellectual disabilities and their families. Our Supports Coordinators engage individuals and their families in the development of plans that ensure their needs and goals are addressed, and life opportunities expanded. Serve and support our clients in developing, maintaining, and nurturing relationships with the important people in their lives. How This is Accomplished Individual plans are developed to assist our consumers as they progress toward inclusion, participating in their communities, and achieving a better quality of life. A bachelor's or associate's degree in a related area of study Driver's license Candidates must have a reliable car 50% travel required (driving to individuals' homes and other locations) At least 2 years of experience working with people with intellectual and/or developmental disabilitiesSalary:
