Job Description

Job Summary Family Care Services is a non-profit human services agency. We are seeking a dedicated and dynamic Life Sharing Program Specialist to provide case management duties to adults diagnosed with intellectual and developmental disabilities living in Life Sharing homes (private residences) in a full-time position. The ideal candidate will possess a strong background in social work, public health, or related fields, with proven experience in working directly with persons with an intellectual disability or autism. No direct care is provided by this position. Potential for advancement. EOE. Responsibilities Coordinating and completing assessments. Writing reports and progress notes. Monitoring clients in homes by doing home visits. Attending meetings. Attending trainings. Facilitating trainings to caregivers. Responding to phone calls and emails. Be a liaison with natural families, caregivers, day programs, and county agencies. Help stabilize clients with challenging or problematic behaviors using positive approaches to behavior management. Participates in incident management. Insures that all of the client's needs are met including physical, psychological, medical, social, recreational, vocational, and safety needs through training and/or appropriate referrals. Requirements Proficient in Microsoft Office Organized Self-motivated Excellent communication skills Detail and deadline orientated Education and experience: Bachelor's degree in human services and 2 years work experience working directly with persons with an intellectual disability or autism. Master's degree in a human services field and 1-year work experience working directly with persons with an intellectual disability or autism. Certified behavior specialist or have the qualifications to become a behavior specialist.
Salary:
$43,000 annually
