(ECCM), a progressive and growing non-profit agency with excellent benefits and competitive salaries, is seeking a talented professional for the following position: A Blended Case Manager (BCM) provides blended case services to adults who suffer from a serious mental illness or emotional disturbance. The BCM's caseload will consist of individuals who are enrolled in Community HealthChoices and receiving Long Term Services and Supports. Therefore, the BCM will be working alongside an assigned Service Coordinator to ensure that participant's medical needs, physical health needs, and socio-emotional needs are all being addressed. The BCM is responsible for assisting their participants in accessing resources to help them remain in the least restrictive setting. In addition to linking with necessary resources and supports, the BCM also fosters independence by educating the participant on resources available within the community to maintain that independence. The overall responsibility of a BCM is to work with the assigned participant and additional necessary parties in order to access, link, coordinate and monitor needed services as well as provide support and encouragement.

Qualifications:
Bachelor's Degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other social sciences, criminal justice, theology, nursing, counseling or education.
