Job Description
Qualifications:
Must have a High School diploma or GED. Previous work experience as a Case Manager preferred. Experience working in the field of addictions treatment preferred. Working knowledge of Medicated Assisted Treatment preferred. Individual must understand and respect everyone's unique path to recovery.Quality of Work:
The Case Manager utilizes a coordinated approach to linking those with substance use disorder to appropriate services. The Case Manager always conducts self ethically and professionally. Understand and adheres to organization, Federal and State regulations, including accreditation agency standard and regulation that govern Drug and Alcohol confidentiality. Training and/or experienced in the recovery process and able to work with community members and agencies is also expected. He/she/they must possess strong organizational skills and can work with the public. Must be able to set up systems that track data collected by the program and have a general working knowledge of operating systems within the drug and alcohol community. Additionally, the Case Manager must possess a valid driver's license and have access to a motor vehicle during working hours. The Case Manager is to spend a minimum of 120 hours per month (or 75% of assigned work schedule) of direct case management to patients. This includes but not limited to, working with individuals in the community as well as existing clients of New Directions Healthcare treatment program, New Directions Healthcare Center of Excellence and or those in need of drug and alcohol treatment. Case Manager offers the person a single point of contact.Responsibilities/Duties and Functions:
Coordinate and facilitate patient care through assessment, evaluation, planning and implementation. Work in conjunction with community resources, CRS services and those that are involved with aiding the client in developing a healthier, recovery lifestyle. Work with judicial system and aid those incarcerated and upon release from prison. Accompany clients to appointments where support is needed. Make referrals to community resources that support healthy growth and stability for clients. Participate in interdisciplinary team meeting and Performance Quality Improvement. Communicate patient needs to a variety of team members and follow up accordingly. Assist with discharge plans upon completion of treatment. Work collaboratively with patients, families, and interdisciplinary team to ensure quality of care. Maintain records of patients as appropriate including needed documentation. Proficient computer skills. Strong written and verbal skills. Duties as assigned.Pay:
