Job Description

The YWCA Greater Harrisburg is seeking a passionate and dedicated full-time Transitional / Rapid Rehousing Case Manager to join our team. This position works directly with individuals and families experiencing homelessness to complete intakes, develop individualized service plans, connect them to community resources, and guide them through the process of securing and maintaining safe, affordable housing. The Case Manager serves as a key liaison between clients, property management, and community partners to ensure program compliance, promote housing stability, and support successful transitions to long-term independence. Position Details:
Hours:
37.5 hour work week; Monday-Friday 8am-4pm ; with a rotational on-call schedule.
Salary:
$20/hr.
Key Responsibilities:
Conduct client intakes, assessments, and individualized goal planning. Provide weekly case management sessions and ongoing advocacy, including referrals and support at external appointments. Assist clients in securing housing, understanding leases, and meeting tenant responsibilities. Deliver crisis intervention and support to promote client safety and housing stability. Maintain accurate client records, data entry, and required monthly/quarterly reporting. Collaborate with property management to ensure compliance with housing and program requirements. Build relationships with community partners and local landlords to expand housing access. Conduct exit interviews and complete required documentation when clients transition from the program Participate in meetings, trainings, and the program's on-call rotation. Perform additional duties as assigned.
Position Requirements:
Bachelor's degree in human services or related field with two years' experience or associate degree equivalent with four years' experience. In-depth knowledge and understanding of the social welfare system. Valid driver's license, clean driving history, and reliable personal vehicle. Proof of insurability may also be required.
