Key Responsibilities:

Provide ongoing support to youth and foster families through regular home visits Develop individualized service plans to inspire emotional, intellectual, and social growth Respond to crisis situations and provide immediate assistance when needed Maintain accurate records in our Electronic Health Record system Plan and attend recruitment events, presenting to community groups and organizations Conduct family interviews and complete home studies for foster and kinship families

Requirements:
