We are seeking a dedicated and compassionate Case Manager to join our team. The ideal candidate will play a crucial role in supporting individuals and families in navigating complex social services and ensuring they receive the necessary resources for their well-being. Responsibilities Conduct comprehensive assessments to identify client needs and develop individualized service plans. Utilize motivational interviewing techniques to engage clients effectively and encourage positive behavior changes. Provide mentoring and support to clients, helping them set achievable goals related to their personal, social, and professional development. Implement crisis intervention strategies when necessary, ensuring client safety and well-being during emergencies. Advocate for clients, ensuring they have access to essential services such as healthcare, education, housing and social determinants of health needs. Conduct case coordination services to identify and ensure that the consumer's treatment and non-treatment needs are being met. Experience A background in social work or related fields is preferred, with experience in case management or direct service provision being highly beneficial. Knowledge of motivational interviewing techniques is advantageous for effective client engagement. Experience working with diverse populations and understanding the complexities of substance use disorders will enhance your ability to serve our clients effectively.
