Full-time | 37.5 hours/week with flexible scheduling to support consumers' needs. Working Monday through Friday, four days a week 11 am - 7 pm, and one day a week 9 am - 5 pm. On-call rotation with stipend reimbursement. About the Adolescent Case Manager (TIP Facilitator) Role The Adolescent Case Manager (TIP Facilitator) is responsible for following the evidence supported TIP Model and the Pennsylvania Blended Case Management Regulations, to support individuals experiencing emotional and/or behavioral difficulties, to achieve their independence goals. The TIP Facilitator works to engage the young person (between the ages of 16 - 26) through relationship development, person-centered planning, and a focus on their future. Complete forms, assessments, and case notes related to the program on time. Achieve billable hours requirements. Ability to prioritize and organize schedule. Participate in 24/7 emergency on call rotation. Available to work evening hours and weekends. Collaborate with multi-systems and family supports. Facilitate community and social events. Provide mediation and crisis support as needed. Qualifications Education & Experience A Bachelor's degree with Major course work in sociology, social work, psychology, criminal justice, counseling. Bilingual in Spanish/English a plus Skills Valid driver's license and good driving record. Ability to work both as part of a team and autonomously.
