Job Description

Job description Provide case management/care coordination and advocacy by carrying a caseload and assisting clients in negotiating social services, providing follow-up and monitoring support. Reassess client need as appropriate.
Responsibilities:
Perform client needs assessment, based on intake materials and client interview Meet weekly with clients enrolled in vocational training programs, to assess their level of satisfaction with the program Assist residents with job applications and applications to vocational and educational training programs as needed Meet weekly with all clients on caseload to address case management issues and concerns including budgeting, entitlement, housing and employment Finalize plans for referrals for therapeutic support
Skills:
Expertise in case management and client advocacy Ability to work with addicted women with critical and emergent situations Sensitivity to cultural and economic differences Knowledge of regional and service pool
Experience:
Thorough understanding of medical and psychosocial issues related to addiction
Education Requirement:
Bachelor's Degree and 3 years experiences in social/human services or Master's degree and one year of related experience
Salary:
Grade 15
FLSA Classification:
