One of our clients is seeking Outreach Case manager in the Germantown area of Philadelphia. Position is 85% hybrid or out in the community. Must have lived experience with homelessness & dependency.

CLIENT'S AVAILABLE HOURS 9

00 am- 5:00 pm Monday - Friday

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY

Conduct comprehensive assessments of clients' strengths, needs, and goals within the first 30 days of enrollment Develop and implement individualized recovery support plans Facilitate support groups, workshops, or educational sessions as needed. Weekly schedule of case management and outreach sessions Other duties as assigned

CLIENT'S REQUIRED SKILLS & EXPERIENCE

Bachelor Degree in criminal justice, psychology, social work, sociology, counseling. 1 year experience as case manager Lived experience with homelessness and dependency Ability to engage with adults dealing with addictions Some experience working in addictions field
