We are looking for an experienced Case Manager to join our team. The successful candidate will be responsible for assessing client needs, developing individualized service plans, and providing case management services to clients in need. The Case Manager will also be responsible for monitoring client progress, advocating for clients in the community, and providing support and resources to ensure that clients receive the services they need. The ideal candidate will have a strong commitment to helping others and a passion for making a difference in the lives of those they serve.
Requirements:
Bachelors Degree or RN/LPN Nursing Degree Advantage Case Management Certification (Preferred, but not required) Must be based in Tulsa or surrounding counties
Responsibilities:
Develop and implement individualized service plans for clients. Maintain client records and documentation in accordance with legal requirements. Conduct assessments to identify client needs and strengths. Provide crisis intervention services as needed. Develop and maintain relationships with community resources to ensure the availability of services for clients. Provide advocacy services for clients and their families. Monitor progress of clients and ensure that goals are met.
