WE ARE CURRENTLY SEEKING EXPERIENCEDD ADVANTAGE WAIVER CASE MANAGERS TO COVER THE TULSA AREA

(Tulsa, Wagoner, Muskogee, Okmulgee, Pawnee, Creek and Payne Counties). AllFaith has expanded our Services to include Advantage Waiver Case Management. We are rapidly growing and hiring experienced ADvantage Waiver Case Managers effective immediately. Full-time and part-time positions are available.

CANDIDATE MUST RESIDE IN OKLAHOMA WITHIN A 75 MILE RADIUS FROM EDMOND OFFICE LOCATION. HOWEVER, YOU WILL WORK REMOTELY FROM HOME. QUALIFICATIONS

One (1) year Advantage Case Manager experience (PREFERRED) ; or RN with one year paid professional experience; or LPN with one year paid professional experience; or_ Baccalaureate degree and one year •paid professional experience with the aging or disabled population obtained before or after receipt of degree. •Paid professional experience may include, but is not limited to: CNA, CMA, CHHA and PCA experience;

RESPONSIBILITIES

Performs the core functions of case management: comprehensive assessment, service planning, service plan implementation, monitoring and reassessment of members receiving Advantage waiver services Participate in and facilitate interdisciplinary team meetings. Develop resources and maintain working relationships within the community to assist in obtaining services for the consumers Assist case manager supervisor with training/orientation of new case managers as needed Utilize Harmony for all services to include service plan development, monitoring activities and documentation Participate in evidence based practice education
