afc case manager Middlesex HealthCare Services Worcester, MA Job Details Full-time $23.27 - $28.02 an hour 5 hours ago Benefits 403(b) matching Mileage reimbursement Qualifications Teamwork Working with individuals with disabilities Social work Bachelor's degree Case management Full Job Description Conduct initial and ongoing assessments of AFC clients' physical, psychosocial, and environmental needs Develop and update individualized service plans in collaboration with members, caregivers, and interdisciplinary team members Perform regular home visits to monitor member health, safety, and caregiver performance Provide education and support to caregivers regarding care techniques, documentation, and program requirements Coordinate services with health care providers, community resources, and other support systems Monitor and document changes in member condition and report concerns appropriately Ensure compliance with state regulations, Medicaid guidelines, and AFC program standards Maintain accurate, timely, and complete documentation in electronic medical records Participate in team meetings, case conferences, and quality improvement activities Advocate for members' rights, preferences, and overall well-being Adult Foster Care (AFC) Case Managers require a Bachelor's degree in social work, psychology, or a related field, along with 1-2 years of experience working with elders or adults with disabilities. They must have strong communication skills, valid transportation for home visits, and knowledge of MassHealth AFC or ready to be trained
