The Assessment Care Manager conducts assessments to determine eligibility for the Massachusetts State Home Care Program and other community-based services, supporting older adults and individuals under age 60 with documented Alzheimer's disease or dementia related illness in accessing resources that promote independence, dignity, and safe community living. Qualifications and Skills Bachelor's degree preferred, with a concentration in social work, psychology, counseling, human development, nursing, sociology, criminal justice, public health, public policy, gerontology, or a related field. At least one year of experience in the human services field is preferred. A valid driver's license, reliable vehicle, and active automobile insurance are required. The successful candidate will demonstrate strong organizational, time management, prioritization, and problem-solving skills, with the ability to manage multiple responsibilities in a fast-paced environment. Must be able to maintain accurate records, pay close attention to detail, document clearly, and communicate effectively both verbally and in writing. The ability to work independently, take initiative, and collaborate effectively within an interdisciplinary team is essential. Computer proficiency, including experience with Microsoft Word and Outlook, is required. Candidates must have the physical ability to climb stairs and lift and carry items weighing up to 20 pounds.
