Work Location:
Hybrid remote in Boise, ID 83706 Responsibilities Case Manager Assess participant needs, strengths, barriers, and available supports. Develop and maintain an individualized Case Management plan focused on identified needs and desired outcomes. Coordinate behavioral health, medical, community, and social services to promote continuity of care. Link members to appropriate providers, community resources, and supportive services. Advocate for participants and assist with navigating systems and accessing needed services. Monitor service delivery and follow up to determine whether identified needs are being addressed. Collaborate with CBRS providers, clinicians, family/support systems, and other members of the treatment team. Support transitions between services and levels of care and promote increased independence and community functioning. Community Based Rehabilitation Specialist-CBRS Provide community-based rehabilitative and skill-building services aligned with participant's individualized treatment plan. Teach, model, practice, and reinforce functional skills related to behavioral health, social functioning, communication, coping, daily living, and community integration. Support participants in applying and generalizing skills across home and community settings. Monitor and document participant participation, skill development, progress, barriers, and response to interventions. Collaborate with the participant, treating clinician, Case Manager, and other members of the treatment team to support continuity of care. Maintain services that are individualized, goal-directed, clinically appropriate, and non-duplicative of other services.

Qualifications:
Pass a background check. Bachelor's or master's degree in social work, Psychology, or a related human services field. Experience in behavioral health and/or case management is preferred, but if you are new to the field and passionate about helping people, we would love to show you how. Strong communication skills and the ability to build rapport with clients. Knowledge of the NASW Code of Ethics and an understanding of confidentiality laws. Above average attention to detail is needed to stay in compliance with all the documentation requirements. Self Drive, a successful case manager is able to manage their time, complete documentation, meet deadlines. Ability to navigate Electronic Medical Record systems. Valid Drivers License Vehicle insurance with Bodily injury limits at 100,000/300,000.
Pay Structure:
This is a Billable Hour position. This differs from an hourly position in that your pay is directly derived from billable services you provide and not just a time in time out.
