Job Description

Cadence Care Network believes in building a cohesive team of managers and staff. We know that our employees are our most valuable assets in delivering quality service to the children and families that we serve. It is our collective goal to evolve, providing quality services to meet the ever-changing needs of the community. A Bridges Liaison provides case management to emancipated foster youth in the Northeast Ohio. Case management involves referring and linking Young Adults in the area that they reside, coaching, educating young adults with Independent Living Skills and Transitional Youth skills. The liaison works within the community, meeting young adults in their home environment, assisting them with locating and securing housing, educating on budgeting, linking to community resources and completing paperwork in a timely manner. A familiarity with child welfare and foster care is helpful.
Required Qualifications:
must possess a minimum of a bachelor's degree in social work, Human Services, Behavioral Health, or a related field. Two years of experience working with families and/or children in the child welfare system is preferred. Shall be an employee of the Bridges provider agency. Possess a valid driver's license. Be twentyone (21) years of age or older.
Preferred Qualifications:
Strong organizational skills, selfdiscipline, & a positive work ethic are beneficial. A preferred candidate would also possess strong detailoriented characteristics as this position involves a high level of independence and decision making. knowledge of SACWIS is a plus.
