Job Description

Under the direction of the Department Supervisor, case manager performs responsible work in determining applicant/client eligibility for services. Work involves interviewing the client, assessing client's need(s), verifying information, and designing a care plan appropriate to client's need(s). Work requires comprehensive knowledge of the community; to serve as the information/referral source to the Department. The employee and supervisor, in consultation, develop the deadlines, projects, and work to be done. Performs other work as requested.
  • Responds to inquiries on the telephone or in person and makes appropriate referral.
  • Conducts initial assessment and reassessment in client's home and/or office; prepares plan of care; makes referrals and monitors client progress.
  • Receives and logs daily and monthly reports; prepares, maintains and submits required reports.
  • Interprets and explains policies, programs, rights and responsibilities to client.
  • Refers client to other County department/community agencies for assistance.
  • Contacts landlords, utility companies, other vendors as client's advocate. Manages Emergency Fund program for department.
  • Performs specified duties in area of assignment.
Experience B.A. Degree in Human Service, Social Services, Business Administration, or related community experience, supplemented by 1-3 years experience (preferably case management) in counseling or related human service field which provides experience in counseling and casework; or an equivalent combination of training and experience. Knowledge of community development and social services delivery programs. Knowledge of social and psychological needs of older adults. Ability to communicate in oral and written format. Ability to work creatively and independently. Experience in working with older adults
