The Care Manager / Assessor plays a key role in supporting older adults' wishes to remain safely in their own homes by assessing needs and identifying eligibility for home and community- based services, resources, and programs.

Job Description:

Key Responsibilities:

Make home visits to assess individual needs through comprehensive evaluations. Develop personalized care plans in collaboration with clients and families. Provide education and support to families about caregiving and resources. Coordinate services and resources, including medical and social support. Monitor progress and adjust care plans as necessary. Advocate for clients' rights and access to necessary services. Document all case activities in accordance with state regulations and agency policies. Investigate reports of aging adults in need of protective services.

• Be a strong consumer advocate who can maintain a balance of objectivity and empathy
• Have excellent communication and documentation skills
• Enjoy working in a team environment providing support to others
• Have advanced computer and documentation skills
• Have a reliable vehicle for work related travel, auto insurance and a valid driver license.
• Pass a criminal background check, child abuse clearance, and possible FBI clearance.
• Pass pre-employment drug and alcohol test
• Must be available to cover on-call hours when required This is a full-time 37.

50 hours per week position, working 8:00am - 4:00pm with .5 unpaid lunch break. Participation in after hours on-call rotation is required.

Work Environment:

Dedicated base office space with flexible hybrid work from home model after successful completion of probationary period (2 days work from home; 3 days in office) to ensure a positive work/life balance Travel throughout Potter County primarily, with occasional travel to McKean, Cameron, and Elk Counties (mileage reimbursed at highest IRS allowable rate)

Salary Range:

$20.34 - $22.95 per hour + on-call stipend, mileage reimbursement at highest allowable tax- exempt IRS rate Minimum education and experience: One year experience in public or private social work and a Bachelor's Degree which includes or is supplemented by 12 college-level credit hours in sociology, social welfare, psychology, gerontology or other related social sciences: or a Bachelor's degree with social welfare major, or any equivalent combination of experience and training including 12 college-level credit hours in sociology, social welfare, psychology, gerontology or other related social sciences: OR Associate degree in nursing, with three years of nursing experience; OR Any equivalent combination of experience and training which includes 12 college-level credit hours in sociology, social welfare, psychology, gerontology or other related social sciences and Two years of aging case work experience including on year of experience performing assessments of client's functional ability to determine the need for institutional or community-based services
